Affiliation will be checked after your complete registration.
* Excluding any pharmaceutical or consulting company staff members.

Please read the terms and conditions below carefully, the following conditions will apply:

- Purchase orders will be invoiced according to the rate applicable at the time of receipt of the order.
- The registration fee is not refundable.
- Physicians or Health Care Professional (HCP) with double affiliation (Hospital salary / private practitioners AND pharmaceutical or consulting employees) should register as industry either partner or non-partner.
  - Only non-industry / pharmaceutical and non-consulting employees attendees can register with the Member affiliation rate (AFEF / Liver Forum / Forum for Collaborative Research).
  - Only non-industry / pharmaceutical and non-consulting employees attendees can register with Resident-Student - Fellow Rate.
  - Non Partner industry registrations fee before the approval of a sponsorship contract can not be refunded.

- Purpose: This data is collected to manage your registration and email.
- Authorization: By the validation of this form you authorize us to use this data for the purpose mentioned above.
- Who has access to the data: Taming HealthCare, Organizing / Scientific Committee, Sponsors and Partner.
- Data retention period: 2 years after the end of the meeting.
- Rights of access and modification of your data: on request at contact@taming.fr
- Transfer of your data outside the European Union: the transfer of data can be useful for Organizing / Scientific Committee, Sponsors and Partner outside the EU.
